The Director will submit a written statement to LPA regarding the actions taken following S1’s rough-handling incident on 12/05/25. The statement will detail steps taken to ensure children’s safety and personal rights, along with corrective measures to prevent recurrence. All required documentation will be submitted by 12/19/25. On 12/05/25, staff S1 was observed handling a child roughly, inflicting pain, which created an immediate risk to the child’s health, safety, and personal rights. Another staff S2 member intervened immediately to stop the behavior.Staff S1 was terminated immediately. 101223(a)(3) · Personal Rights. To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.
Licensee states that staff/teacher(s) shall be required to attend a training class provided by a parent educator with proof of attendance that will be submitted to Licensing. Licensee will also submit to LPA a written plan of action indicating understanding children's personal rights. Based on interview and record reviews, S-1 was dismissed due to inappropriately communicating with children in care, which posed an immediate health, safety, or personal rights risk to persons in care.

101229(a)(1) · Responsibility for Providing Care and Supervision The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.
Director will submit a written statement of her understanding of CCR 101229 and will review with staff CCR 101229, proof attendance will be sent to The Regional Office by close of business. Based on interview,record review and observation, c1 and c2 were not supevised during care, which posed an immediate health, safety, or personal rights risk to persons in care.

During today's inspection LPA Macias requested the following documents; Self-Certification LIC9275, Sampling Checklist Form LIC9276, Facility Sketch LIC 999, fully labeled with locations of all water outlets, and full lead report, to be submitted by the POC date. The lead exceedance reading was found in a faucet (J-6.4) in the yard/ play area. This poses a potential risk to the health, safety, and personal rights of the children in care. During today's inspection LPA Macias observed the affected faucet is not utilized by children, and is labeled. The posting reads lead exceedance found, this faucet is not to be used for consumption.
