Per facility Director, staff #14 left to take another job. Per Director Wong a training will be conducted with all staff regarding personal rights and the well being of children/ peronal rights using CCLD videos on website. Documentation of the training will be submitted to LPA Perry by POC date.
Staff #14 placed hands on children face to redirect the children which poses a potential risk to the health and safety of the children in care. Declarations will be submitted by all staff regarding what they learned.

Interviews were conducted with Staff 1 (S1) to Staff 5 (S5). Of the 5 Staff interviewed, Per Director, additional training for Staff will be provided pertaining to Supervision of Children in care. Proof of training will be forward to LPA once completed. 1 Staff member was able to corroborate the allegation indicating that they witnessed a Staff member not providing supervision to a child which led to a fall. LPA reviewed facility report (date 11/22) which documented the fall and that ice was applied to the injured area.
