During file review and LPA Licensing information system review Director handed LPA a copy of staff #10 transfer request form, LIC 508 and a copy of her drivers license to have the staff member associated to the facility. Staff #10 was verified by the department to be cleared in guardian. it was observed that staff #10 has not been associated to the facility and has been working since 08/2021. This poses a potential health and safety risk to the children in care. A $100 civil penalty will be assessed on this date.
