Deficient Practice Statement During review of records, it was discovered the the Licensee did not have any required licensing forms on file for any of 6 children that were present during the inspection. Licensee stated that she provided the parents with new forms to complete as a lot of the families had moved, however per the licensee, the children have been enrolled from about 2 months to 2 years.
