interviews conducted and record review While LPA was present, the school age child was picked up by their guardian. Licensee will write a letter stating that she agrees to operate within the limitations of the license. Licensee will provide letter to LPA before leaving. It was revealed that the facility is providing care to a school age child (7 years old) without a school age license. This poses an immediate health, safety and/or personal rights risk to children in care.

record review and interview A review of Guardian shows that Adult #1 has been fingerprint cleared and associated to the facility as of 3/25/23. Deficiency cleared. Information obtained from the interviews. During interviews Adult #1 was identified as being a staff member at the facility. LPA observation. During today’s inspection LPA observed outside a big wheel toy with both During the inspection the toy was moved to an area which is inaccessible to children. Director stated she will have the missing door knob fixed and provide proof to Community Care Licensing by pedals missing. LPA also observed a door with a missing door knob.
