by Director will train staff on YMCA standard operating procedures which include all aspects of safety and supervision. Based on staff interview and UIR submitted, the licensee did not comply with the section cited above. C1 was left alone on 11/15/2024 without any adult supervision inside the bathroom for 3 to 4 minutes, which poses an immediate health, safety or personal rights risk to persons in care.
