101416.2(b) · (b) Prior to employment, an infant care teacher shall have completed, with passing grades, at least three postsecondary semesters or equivalent quarter units in early childhood education or child development, and three postsecondary semester or equivalent quarter units related to the care of infants, at an accredited or approved college or university.

Facility agrees to submit a written statement acknowledging S1 can not work alone in the infant classroom without the necessary units and what their plan will be to cover staff breaks. Facility agrees to submit written statement to CCL by 9/8/25. Based on interviews and record review, S1 was working in the infant classroom alone without the necessary units, which poses a potential health, safety and personal rights risk to children in care.

101430(a)(3)(E) · (a) Notwithstanding Section 101230, the following shall apply (3) All infants shall be given the opportunity to sleep without distraction or disturbance from other activities at the center whenever the infant desires. (E) If an infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible.

Facility agrees to provide in-service training to staff regarding the Infant Care Activities and submit the training agenda along with staff participation list to CCL by 9/4/25. Based on LPA's observation, an infant was seen sleeping in a high chair, which poses an immediate health, safety and personal rights risk to children in care. 101220(a) · Child's Medical Assessments (a) Prior to, or within 30 calendar days following the enrollment of a child, the licensee shall obtain a written medical assessment of the child. This medical assessment enables the licensee to assess whether the center can provide necessary health-related services to the child.

Deficient Practice Statement Based on record review, the facility did not comply with the section cited above in 2 out of 4 children. 2 children did not have a physicians report on file. This poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 01/19/2024

101238(a) · 101238 Buildings and Ground (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.

Facility will submit pictures of washed trays and refrigerator to the department by POC due date 5/23/23. Based upon LPA observation and photos obtained on visits 04/26/23 and 05/19/23, the facility did not meet the section cited above in that infant trays for storing food and outside of infant refrigerator had built up dirt and debris. This is a potential health and safety risk to children in care.
