The Licensee stated, during the Licensee and LPA Estoesta discussion over the phone, that S1 and S2 no longer works in the facility starting 5/21/2025. On LPA's 6/3/2025 visit, LPA confirmed that S1 and S2 were not present in the facility. The Licensee will update the Guardian Roster and will submit an updated LIC 500 that will reflect the current employees that are actively working in the facility. The Licensee will submit a POC proof to the Regional Office on or by the POC due date. LPA encourage the licensee to reach out to the LPA as needed.

  On LPA Estoesta visit conducted on 05/20/2025, S1 and S2 were present on this visit and did not obtain a California clearance or or a criminal record exemption as required by the Department. Based on the LPA's evidence, the licensee did not comply with the section cited above which poses an immediate health, safety and personal rights risk to children in care Civil penalty is assessed today.
