Uncleared individual was removed from the facility. Licensee was advised that individual cannot continue to work in facility until she gains fingerprint clearance. Civil penalties were assessed in the amount of $100.00 Based on observations and record review the facility was found to have an uncleared indvidual on staff since September 2021.
