Licensee stated that they will submit proof to licensing by POC due date. it was revealed that Child #1 was handled in a rough manner by Staff #1. This poses an immediate risk to the health, safety, or personal rights to children in care. Licensee stated that they will conduct a staff training on how to have a positive relationship between staff and children during operational hours. Licensee stated that they will cover strategies on active supervision plan. Licensee stated that they will submit proof to licensing by POC due date.

101238.2(e) · Outdoor Activity Space. All playground equipment shall be cushioned with material that will absorb a fall. During today’s inspection LPA observed there was inadequate wood chips underneath the slide that revealed an exposed tree root. This is a potential risk to the health, safety or personal rights of children in care. Director has ordered additiional wood chips which are expected to arrive in approximately three weeks due to a shipping issue.
