Director stated that they have already created an action plan and completed a supervision refresher at their staff meeting and will provide proof of completion to LPA via email by 8/29/24. Based on staff interviews, C1 was not properly supervised leading to C1 being left unattended in the classroom for approximently a few minutes. Center Manager Rachel Smith advised they had a Staff Meeting on Active Supervision, plus they all watched a CDSS video about active and dual supervision. They redid the rosters, so it's face to name recognition instead of just head counts. Teachers have implemented playground rotation. Managers will randomly go outside and check on The facility submitted an Unusual Incident Report dated September 6, 2022, which reported that Staff #1 (S1) observed Child #1 (C1) was left unattended on the playground, after C1's classroom returned indoors, which poses an immediate health, safety or personal rights risk to persons in care. children and teachers, Also, staff were advised if they are having challenges they can use the walkie talkie to call for assistance. Documentation and sign in sheets provided to LPA during today's visit. A zero tolerance civil penalty will be issued and Mrs. Smith will ensure the LIC 9224s are completed within 24 hours.
