Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. 2 out of 2 infants have incomplete sleep logs. LPA observed that the last documented sleep was in July of 2025. Licensee was on vacation 9/1/25 - 9/12/25 so there is no sleep log for September. POC Due Date: 09/30/2025
