Declaration to be submitted to to LPA, via email, by 7/18/25. The staff have already installed latches the main gate and the gate to the yard. Also an alarm has been installed to the yard gate. A security company has been contacted to assess children safety.

Child #1 was found wandering outside of the facility. Staff admitted that Child #1 was found outside the facility, pass the parking lot and returned by two passerby. Child was found safe. This poses an immediate risk to the health, safety, or personal rights of children in care The director has met with the teacher regarding the incident, supervision, headcounts, assigned yard zones and document to note number of kids. Director will draft a document and have the staff sign they understand. Staff will review videos the the Department's website. Proof will be submitted via email.
