The incidents with the substitute staff were reported to the outside agency that was used and the substitute has been banned from any facility in this program. A memo will be created for new staff regarding classroom procedures and expectations and children's personal rights. Based on interviews S1 &S2 witnessed a sub staff put their leg on a child during nap time while the child stated they couldn't breathe. Sub Staff was immediately told to stop. This poses an potential risk to the safety and personal rights of children in care. Memo will be submitted by 11/7/25.

The Director and Site Supervisor took immediate action and removed Staff #1 who is permanently blocked from employment at the center. Director took appropriate action and reported to CCLD licensing, and conducted their internal investigation and completed the required reports. Talked to parent about the incident. Based on interview and evidence collected, the licensee did not comply with the section cited above on 02/23/24 the center observed surveillance video which substantiating the incident Staff #1 to Child #1 which poses/posed a potential health, safety or personal rights risk to persons in care.
