101229(a)(1) · 101229 Responsibilty for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement has not been met as evidenced by Director to provide the Department proof by POC due date to show a schedule that has a fully qualified teacher in each classroom. Director to provide a written plan on how to review educational qualifications for future employees to ensure facility compliance. LPA observed four out of the five classrooms without a Fully Qualified Teacher present. This poses a potential risk to the health, safety and personal right of children in care. Director to provide to the Department by the POC date proof that all staff have valid Mandated Reporter training on File. Director to provide a written plan on how to track and maintain compliance for required training for all staff.

Prior to today's inspection the facility self-reported by submitting an unusual incident report, the facility immediately stopped use of fountain and had it repaired on 09/01/2022.
