Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During the indoor inspection, Licensing Program Analyst (LPA) observed dark staining in different areas of the brown carpet as well as staining on the colorful carpet used for circle time in the 4s classroom. LPA also observed several holes and frayed areas in the brown carpet. During the outdoor inspection, LPA observed that one of the wooden boards toward the bottom of the fence was broken, leaving a splintered area. POC Due Date: 11/01/2024
