Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. During the indoor inspection, Licensing Program Analyst (LPA) observed that the colorful carpet used for circle time was stained with dirt marks throughout. LPA observed a floor mat near the front door with heavy staining. In addition, LPA observed that the AC vent in the wall had a thick layer of dust. POC Due Date: 11/14/2025
