(1) No child(ren) shall be left without the supervision of a teacher at any time... specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. The Site Supervisor held a meeting with all staff on the day of the incident in which they discussed strategies on how to ensure that children are not left without supervision. They also made a safety plan for when bringing children indoors or taking them outdoors. Plan of correction completed and cleared.

Based on interviews, staff confirmed that a child was left unsupervised on the playground for about 30 seconds while everyone went back inside of the classroom. This poses a potential health, safety, or personal rights risk to children in care.
