(1) No child(ren) shall be left without the supervision of a teacher at any time... specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. The Site Supervisor held a meeting/training with preschool staff on 09/03/25 in which they developed an action plan, reviewed active supevision strategies, and made changes to their protocol for when bringing children indoors. Plan of correction completed and cleared.

Based on interviews, staff confirmed that a child was left unsupervised in the outdoor play area for about seven (7) to eight (8) seconds, while everyone went back inside of the classroom. This poses a potential health, safety, or personal rights risk to children in care.
