101238(a)(1) · Buildings and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. The licensee shall take measures to keep the center free of flies, other insects, and rodents. The Director will schedule Ecolab for a follow-up visit to assess the situation in the infant playground and storage area and to outline an approach to eradicate the rodent concern within the facility. During the interview with the Director and Assistant Director, it was disclosed that on 11/4/25 rat droppings were found in the preschool playground and on 11/10/25 additional droppings were discovered inside the teachers’ lounge. According to the Director, the facility has an ongoing issue with rats, and Ecolab provides treatment services twice a month. During today’s visit, the LPA and Director toured the infant playground and observed rat droppings in the storage area near the infant playground. This poses a potential health, safety or personal rights risk to persons in care (POC) will be submitted to CCL by the POC due date.

Per director, Lauren Friedman going forward is not to bring any child into the kitchen. A different appliance for heating bottles that is smaller and can sit far back on the counter was purchased. Based on staff#1 and staff#2 interviews and unusual incident report that was submitted on 05/05/23, child#1 got red mark on 05/03/23 on left arm and was diagnosed with the first degree burn by a Doctor. This poses an immediate health, safety and personal right risks to the children in care. $250 Civil Penalty was assessed today for repeated violation within 12 months.

Per director, she will not use bouncers in the facility anymore. Per director infants will sleep on the crib or cots based on their age. Based on staff#3 and staff#4, there were times that while infants were playing on the bouncers fall asleep and slept on the bouncer about 10-20 minutes and then a staff moved the infant from bouncer to the crib. This poses an immediate health, safety, and personal right to the children in care. A written plan will be submitted to LPA Eivazian by 05/02/23.

101439(d)(2) · Infant Care Center Fixture, Furniture, Equipment and Supplies: A baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Section 1596.846. This also includes baby bouncers.
Per director, she did not know bouncers are not allowed. However during this inspection director took out all three bouncers from the facility. A written plan will be submitted to LPA by 05/02/23. On 05/01/23 LPA Eivazian observed two bouncers in Hives classroom and one on outdoor playarea. LPA observed an infant on bouncer playing. This poses an immediate health, safety and personal right risk to the children in care.
