evidenced by facility not providing Per Facility Director Veronica all staff will review reporting requirements and Director will send statement of acknowledgement to the LPA by POC date. Director will hold staff meeting to review how to report and complete an injury report and how to notify the parents with documentation from incident reports as staff were not diligent in record keeping, additionally for not reporting to the department child injuries. Based on the information obtained Facility does not report unusual incidences, which poses/posed a potential health, safety, or personal rights risk to persons in care. authorized representative and record in the child's record and include the date and time of notification and the name of the qualified staff person making the notification. irector is also adding a signature line to incident reports for parents to sign.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified Director stated that staff will be provide additional training (Section 101229 Responsibility for Providing Care and Supervision) to all staff. Supervision shall include visual observation.
Per interviews with Staff and parents, it was stated that children have unexplained injury’s due to the staff not observing what happened. Though no children received medical attention, a lack of supervision occurred child-care-centers/ videos and provide a summary of what they learned and provide proof to LPA by POC date.
