During inspection LPA Perry observed toy removed and placed in preschool yard. Per Director and Site Supervisor they will review all toy label descriptions for age approperiateness. Director and Site Supervisor will submit plan in writing on LIC 855 by POC date to LPA Perry. Based on observation, the licensee did not comply with the section cited above, toy in the infant indoor outdoor play yard was not observed to be age approperiate, which poses a potential health, safety or personal rights risk to persons in care.
