facility tour on 03/29/22. LPA observed uneven pavement in the play yard. There was an inoperable children's bathroom stall and there was paint During today's visit 06/16/2022, LPA observed all the children's bathroom stalled to be in good repair and LPA also observed the all uneven pavement was removed and replaced by new turf. Pictures were taken by LPA. Per Director will have paint chipping fixed and send LPA pictures by POC due date of 07/07/2022. chipping above the refrigerator where food is prepared for children in care. This is a potential risk to the health and safety of the children in care.

101212(d) · Reporting Requirements: Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in Per Director, will review the reporting requirements and will report any future incidents to the department within the 24 hour period.
