Per the site supervisor, upper management may revisit the facility procedures in place to allow site supervisor to notify parent or authorized representatives of any allegations and administration will conduct a follow up with parents/authorized representative. Per the site supervisor, the parent handbook will include the right for the parent to be notified on the same day of any incident or allegation where the child is involved. An updated parent handbook will be submitted to LPA by POC due date. Based on interviews it was revealed that the facility did not immediately report an incident to the parent or authorized representative of C1 due to the facility protocol.

101212(d) · 101212 Reporting Requirements (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.
