Water Sampler Self-Certification form of LIC 9275, Child Care Center Sampling Checklist form LIC 9276, Facility Sketch LIC 9099 (water faucets/outlets identified) completed. Faucet T replaced and is following next appropriate steps as outlined in written directives.
