Director will submit a sign in sheet of training including title of training, date, printed staff names, staff signatures, and a brief summary of training to LPA via email at: ranita.richmond@dss.ca.gov. On 01/30/26, a UIR was called in to the El Segundo Childcare Regional Office by the director of the above named facility stating that a child was served a meal that contained foods that the child was allergic to. The child's allergies are documented in child's files and inside the class room. Director will create a daily food allergy list to include the following: Name of child with food allergy, picture of child with food allergies, food served, food ate, teacher child ate with signature, date, meal type (breakfast, lunch, staff). Director will submit copy of food allergy check list to LPA via email.
