On 10/17/2022 at approximately 12 noon, Child #1 was found Staff training was conducted on 10/19/2022. LPA was provided with a copy of the traning agenda, staff sign in sheet, and list of safety measures that facility has implemented. Facility staff had training topics such as Personnel Policies, Sanitation, Safety and Supervision. outside of the school grounds. The child was discovered by the school attendant on the sidewalk on the corner of Willoughby Ave. and Edinburgh Ave. Staff members were not aware that child had exited the school grounds.
