Per Director, prior to LPA leaving 4/12/24 a snack menu will be posted and a copy of menu will be sent to LPA via email. A written plan will be submitted to LPA on how they can ensure menus are being posted accordingly by POC date. Based on observation, record review and interviews, LPA did not observe snack menus posted and it was disclosed by S#1-S#5 that snack menus are not posted accordingly, which poses a potential health, safety or personal rights risk to persons in care.
