Per Principal, they will meet with staff regarding supervision and head injury protocol. Written plan on how to maintain supervison, Meeting minutes and sign in sheets will be provided to LPA by POC due date of 3/13/2026. Based on observation and interviews, the licensee did not comply with the section cited above in C2 fell causing a fractured arm in an area that was not supervised and not all staff was informed of C3 head injury for appropriate supervision. Which poses/posed an immediate health, safety or personal rights risk to persons in care. LPA is requesting that Principal meet with all staff in regards to personal rights. Please submit proof of meeting via email to LPA. Based on interviews conducted with C2 and C4, C2 disclosed that S4 grabbed them by the arm and pulled them. C4 demonstrated to LPA how S6 kicked and stated that it hurt when the incident occurred, this poses/posed a potential health and safety risk to children in care.
