Director agrees to submit a staffing plan to LPA via email ensuring proper ratios are maintained. Director will disscuss staffing plan with all preschool staff and submit statement of understanding to LPA by 10/3/2025.
At approximately 1:38 pm LPA Laureano observed the two-year-old classroom with 17 sleeping preschoolers and two children who were awake with one teacher. Citations was corrected by the end of the visit

101223(a)(1) · 101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights:(1) To be accorded dignity in his/her personal relationships with staff and other persons. Director will conduct a staff meeting to review Personal Rights regulations and review training videos on personal rights. LPA will provide the link for the training. Director will submit an attendance sheet for the staff meeting to the LPA by 08/14/2025.
During the complaint investigation interview, it was revealed that Staff 2 yelled at children in care Director will complete and submit a signed declaration stating that S2 is no longer at the facility. Declaration was collected during the inspection.

Director agrees to provide staff with memo regarding wellness checks being performed in the morning, mid-day, and before going home in order to determine any injuries children may have sustained that they are not aware of. Based on observation, interview and record review, facility staff were unaware of the injuries sustained by C1 while in care, which poses a potential Health or Safety, or personal rights risk to persons in care. Director will provide copy of memo with staff signatures indicating that they have received memo.
