  based on record review and interviews facility failed to provide adequate care for C1 during outdoor play. C1 was placed on the hook which caused the hook to break, which poses an immediate health and safety risk to children in care The facility conducted staff training on 2/13/26 covering care and supervision and children’s personal rights. LPA obtained the training materials and sign-in sheet as proof of attendance. POC was cleared during today's visit.
