Based on video footage and interviews, S4 did not ensure supervision was maintained at all times which caused C1and C2 to wander to another classroom without supervision, which poses an immediate risk to the health, safety, and personal rights of children in care. Per Director, they will be scheduling a in person training with Technical Support Program to review supervision and discuss how supervision needs to be maintained at all times with facility's staff members. Based on video footage and interviews, facility did not ensure supervision was maintained at all times which caused C1 to be left without supervision in the bathroom and classroom, which poses an immediate risk to the health, safety, and personal rights of children in care. Meeting minutes with signatures will be send to LPA by POC date. Based on interviews and video footage it was disclosed that staff were not responsible for providing care and supervision for children in care. Based on interviews and Per director, S1 has been placed on administrative leave. Per director there was an in service meeting with all staff on 05/24/2024 reviewing supervison and personal rights. Director will submit sign in sheets of staff that attedned to LPA by POC date (06/07/2024). video footage review. S1 was observed to handle C1 in an inappropriate manner.
