Staff will provide opportunity for infants to rest without interference. Director will conduct a training to ensure staff are allowing infants to sleep undisturbed. Director will provide a sign in sheet of staff who attended training for department to review. LPA observed infants being waken up from napping at 10:08am and 10:19am by staff. Infants were unable to sleep without interference from staff.

Staff will lay infants down on their back to sleep and document sleep position to match daily on infant sleep chart as necessary as the infant sleeps. LPA reviewed infant sleep charts of the infants under 12 months of age. LPA observed 6 infant sleep charts of children under the age of 12 months. LPA observed on 6 of 6 infant sleep charts, that infants under the age of 12 months are placed in a position other than their back to sleep.

Director/ Staff will observe and sign LIC 9227 form in section D no later than 24 hours from observation of infant mastering the ability to roll from their back to their front and their front to their back. LIC 9227 form will be placed in infant files for review by the Department. LPA observed No signature for provider in Section D of LIC 9227on 5 of 7 forms. LPA observed 2 of 7 infant files to be missing LIC 9227 forms. No LIC 9227 forms for two children under 12 months of age . LPA observed infant two (2) children sleeping on their stomach during resting time. Director agrees to have Staff #3 cleared and associated to facility before Staff 3 is allowed back in the facility. The Director will notify LPA via email when it has been completed. Based on observation, record review and information obtained the facility did not comply with the section cited above, 4 children were left under the supervision of an uncleared staff (S3) during today’s visit from 9:00am to 10:54am., which poses/posed a potential health, safety or personal rights risk to persons in care. Director agree to develop and implement a plan to make sure that they are compling with the capacity requirements throughout the day for an Infant Facility. Director will submit the developed plan in writing to LPA via email by the POC due date. Based on observation, the facility did not comply with the section cited above, between 1:43pm to 2:00pm LPA observed only one staff providing care and supervision for 5 infants; two of which were awake and the other three napping, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the facility did not comply with the section cited above, at 1:10pm LPA Castellanos observed a loose blanket and a plushy blanket inside the crib with the sleeping infant (C2), which poses/posed a potential health, safety or personal rights risk to persons in care. Additionally agrees to have all staff watch the following webinar: Based on observation, the facility did not comply with the section cited above, at 1:09 p.m. in the napping area LPA observed two infants (C4 & C1) out of 4 infants sleeping with oversized sleeping sacks, which poses/posed a potential health, safety or personal rights risk to persons in care. https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep will submit a written summary of what was covered to LPA via email by POC due date.

Based on observation, the facility did not comply with the section cited above, at 1:15 p.m. LPA observed all the infant’s bottles and containers on the classroom’s counter not to be labeled with the infants’ name, which poses/posed a potential health, safety or personal rights risk to persons in care.
