In addition facility will review title 22 regulation with all staff on Responsibility for providing care and supervision and have staff sign and date the form to ensure they understand. Deficient Practice Statement Based on observations, interviews and record review, the licensee did not comply with the section cited above in C#7 was observed to have expired medication which was observed on the prescription label which poses/posed a potential health, safety or personal rights risk to persons in care. child #1 exited through the emergency exit door with no supervision in the preschool yard, which poses an immediate risk to the health and safety of the children in care Cynthia Meza, Director also stated that the latch to the door was made so it would be inaccessable to children and this was completed on May 22, 2025. LPA did observe the addidtion of a metal covering. Per director for on going supervision staff will be communicating to eachother on adjusting to the childrens choices on areas and providing 100% supervision in whatewver area they are in.

Per Director, Cynthia Meza, all staff will watch the video "Child Care Reporting Requirements" at ccld.childcarevideos.org and will submit a summary of the video to LPA. Cynthia Meza, Director will submit LIC624 form to report the incident. Based on interviews and record review, the licensee did not comply with the section cited above as the facility did not report to the Department the Unususal Incident of child #1 exited through the emergency exit door with no supervision in the preschool yard, which poses/posed a potential health, safety or personal rights risk to persons in care.

staff interviews. S1 left C1 in the classroom alone, unsupervised Per director, on 10.23.2023 training on Supervison on inside and outside classroom. Along with video training on transition tracking. With the specific staff they have additional individualized trainings. Director will send meeting agenda via email and include LPA on email. POC due date 10/20/21 member left a child in the classroom alone, unsupervised. Child was found by another staff. This was a potential risk to the health and safety of the child in care.
