Per Director, they had a staff meeting to review Personal Rights, lifting and holding policy, and nonviolent coomunication. Signed attendance sheet and copy of meeting agenda were provided to LPA. Per Director, S4 is no longer employed by the facility. Based on observation and interview, facility did not sure that a child was accorded dignity, as camera footage showed S4 violating C1 personal rights in two different occassions. Based on the information obtained unuslal incident that happend on June 11, 2024 was not reported to the department in a timely manner which poses a potential risk tot the health and safety of the children in care. Per Director, they will conduct a staff meeting to review supervision and discussing how supervision can be maintained at all times. Staff will watch video titled Supervising Children in Child Care Centers on www.childcarevideos.org and provide summaries of their understanding Based on interview, licensee did not ensure supervision was maintained at all times at the facility. Based on interviews with S#2 and S#4 neither were positioned close enough to attend to C#1 while child one was climbing the structure, which resulted in C#1 obtaining a fracture on the leg, which posed and immediate risk to the health, safety, and personal rights of children in care. of what supervision is in the childcare center. Director will submit staff meeting agenda, signatures form acknowledging each staff members attendance, and copies of the video’s summaries, to LPA by POC date. Director will remove the climber in Lilac classroom and sign declaration by POC date of 3/15/2024.

Per Director she will have a staff meeting to discuss child’s personal rights and will watch personal rights video on https://ccld.childcarevideos.org/ Director and staff will write a declaration with plans implemented for this not to happen again. Declarations will be submitted to LPA via email by POC date. Based on manufactures information, it was determined that facility utilized the climbing structure outside of the manufacturer's instructions. As well as not treating the child healthful.
