    Per Acting Director- a plan on how to provide assistance and extra activities for children will be done. Per Director a staff meeting/training will be conducted with teachers. A copy of plan and minute meetings will be provided by POC to LPA Lopez via email. Based on LPA’s observations and interviews conducted Staff did not provide a safe environment resulting in C2 who had previous similar incidents causing C1 injuries. This poses a potential risk to the health and safety of children in care.
