Per Acting Director- a plan on how to provide assistance and extra activities for children will be done. Per Director a staff meeting/training will be conducted with teachers. A copy of plan and minute meetings will be provided by POC to LPA Lopez via email. Based on LPA’s observations and interviews conducted Staff did not provide a safe environment resulting in C2 who had previous similar incidents causing C1 injuries. This poses a potential risk to the health and safety of children in care.

Per Acting Director: Contract has been revised- an updated contract will be given to parents and acknowledgement will be sent. Proof of acknoledgement will be provided to LPA via email by POC due date. Based on Lpa’s observations and interviews conducted C1 was disenrolled without 15 days prior written noticed given to parents as stated in enrollment agreement. This poses a potential health and safety risk to children in care.

Per Acting Director the two extra toiltets will be blocked off to prevent children from using them. This will leave the original two toilets available for children to use.
