the incident which occurred Assistant Director states the staff was written up and a training was provided to all staff on 10/21/22. LPA was provided with a copy of the agenda. LPA will be sent a copy of the staff sign ins by POC due date of 10/27/22. LPA was provided a copy of the staff write up for S1. on 10/17/22, when S1 fed an infant the incorrect breast milk.This was an immediate risk to the health and safety of the infant in care.
