Director provided a written statement regarding that she counseled each staff member regarding supervision and moving forward a permanent staff member will always be present with a substitute teacher from an outside resource during nap time and during the day to ensure supervision at all times Based on LPA’s interviews, 2 out of 2 staff stated the incident regarding S1 walking outside of toddler classroom while toddlers were napping for approximately 1 to 2 minutes. This action poses a risk to the health and safety to children in care. Director called a pest control specialist on 08/29/2023 to conduct a rodent inspection. Director provided rodent inspection invoice to LPA Lucero. Director placed child proof mouse traps outside facility entrance gate. LPA observed mouse traps. Based on interviews with 5 staff members, it has been determined there was a mouse at the facility. This poses a potential health and safety risk to children in care.

Baesed on observation on 09/05/2023, a window screen was partially torn open from.. During visit on 10/13/2023, LPA observed window screen in good repair. the bottom right corner. Window is located above foam climbing block equipment that is used by infants. This poses a potential health and safety risk to children in care.
