Executive Director states that a pull lock mechanism was installed front gate. Staff will review supervision and action plan. The center will implement a new face-to-name count sheet that will be implemented when going out to the play yard and returning inside the classroom. Based on surveillance video footage of 08/11/25, the staff did not supervise a child that wandered off which poses an immediate health, safety and/or personal rights risk to persons in care.
