Deficient Practice Statement Based on observation and record review, the licensee did not comply with the section cited above. During tour at 2:00pm, LPAs observed 2 staff that were not on the Department roster. At 2:20pm, Director provided proof of clearance. At 2:21pm, LPA Sanchez call the Regional Office for more information. Staff #2 and Staff #3 are not cleared. This poses/ posed a potential health, safety or personal rights risk to persons in care.
