LPA was advised by owner that the staff member responsible is no longer employed at the facility. Director states that she will complete a declaration (LIC855) indicating that staff members have met and reviewed regulations regarding personal rights of children in care. Based on interview and record review, the director did not ensure that each child is accorded dignity in the children's personal relationship with staff, which poses a potential health, safety and/or personal rights risk to persons in care.

Director states that she will complete a declaration (LIC855) indicating that staff members have met and reviewed regulations regarding personal rights of children in care. Based on observation, interview and record review, the director did not ensure each child to be free from corporal or unusual punishment, which poses and potential health, safety and/or personal rights risk to persons in care.

Director states she will advise infant teaching staff to not use inclined rockers or swings to assist with napping an infant. Director states rockers and swings will only be used while infants are awake. Director states she will provide a declaration - LIC 855 indicating what infant rockers and swings will be used for and/or remove equipment completely. Based on observation, interview, and record review, the infant teachers did not provide a crib or portable crib meeting United States Consumer Product Safety Commission safety standards which poses a potential Health and Safety and/or personal rights risk to persons in care.
