POC: On 6/12/26, facility Director submitted plan of action, which details a procedure to provide medicine to children. Based on interviews, and record reviews, the facility did not comply with the section cited above, as C1 was administered wrong medication by staff member, which poses a potential health and safety risk to persons in care. Since the facility has opt to repair/replace the water outlets, a follow up water sample shall be taken following the lead testing protocol. Licensee failed to maintain a lead value at or below the Action Level for water lead testing with values of 5.5 ppb or greater for Oak classroom sink, which poses a potential health and safety risk to persons in care.
