Created a "Name to Face" roster for each classroom. In the morning and playground time, teachers will utilize the "Name to Face" roster. Director and teachers will confirm completed roster. Based on the Unusual Incident Report, interviews, and record review the facility did not ensure C1's supervision when transitioning from outdoor play back to the classroom. This poses an immediate risk to the health and safety of children in care. Director will conduct training and role playwith staff for supervision and transition.

Director will have professional services manage pest control. In addition, director will create a log to document services rendered. During inspection of the kitchen area, insect and/or vermin droppings were observed. Roach eggs or insect eggs/larvae were also observed. No live insects, vermin, or rodents were observed in the kitchen area.
