Observation Site Supervisor indicated that she had already submitted a work order to the maintenence person on site, but has not obtained status on the repair date. Site Supervisor will follow up and provide a photo of the repaired side board panel by POC date: 11/3/2021 During the inspection of the play yard, LPA observed that there was rotted pealing wood paneling on the outisde of the preschool buidling and was exposing rusted nails.
