These rights include, but are not limited to, the following Licensee stated is in the process of hiring another assistant to ensure there is active supervision at all times. Will place assistants in accessible rooms (family room, living room, dining area and outdoor). There will be a total of 3 staff present providing care and supervision. (1) To be treated with dignity in his/her personal relationship with staff and other persons. (1) The licensee shall request the child's parent or authorized representative to sign and date the bottom portion of the notice form LIC 995A (8/06), which acknowledges that the parent or authorized representative has received and read the LIC 995A.
