Director/Licensee states that both staff 1&2 were retrained on 10/10/25. Staff will be positioned on playground at 11am and 5pm for supervision. Director/Licensee states that he will also conduct training for all staff on 1/24/26 for supervision and personal rights. Director/Licensee will provide proof of completeion to LPA via email 1/26/25.

101229(a)(1) · (a) The licensee shall provide care and supervision as necessary to meet...(1) No child(ren) shall be left without the supervision of a teacher at any time except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.
Director states the children will be counted before they enter the facilty after van pick up. The children will stand in one line until they are cleared to enter the facility. Director states they now physically check the van to ensure no children are left behind. The ratio of the children will be given to the teacher. The directir will send declaration to LPA. Based on interviews conducted, staff failed to ensure that all children were removed from the van after school pick up. C1 was left alone outside the facility sleep in the van which poses an immediate health, safety or personal rights risk to persons in care.

All staff will be train on incident reporting in reference to CCL requirements. Licensee will provide proof of training to LPA via email no later than 05/30/2025. Licensee will provide their internal procedures on reporting incidents to their management and CCL. Based on interviews and record reviews the licensee did not ensure the above allegation that C1 was Left inside of a parked vehicle was reported to CCL, which poses a potential risk to the health safety and personal rights of the clients in care.
