Facility will conduct a staff training on Children's Personal Rights in Child Care on the CDSS website and send proof of Personnel Sign in Sheets of the training to LPA Tamayo no later than 07/11/2025 for proof of completion. Staff initially advised for child #1 to not hang on the rails, however staff did not intervene or intercept the child from hanging on the slide rails, which resulted in child #1 sustaining a one inch laceration to their head. This poses a potential health and safety risk to day care children in care.
