Based on interviews and a review of video footage, S1 admitted to grabbing C1’s chin and speaking to them in a harsh tone. S1 grabbed C1 on their lap and did not allow the C1 to get up when they attempted to do so. This poses a potential health and safety risk to the children in care.

101216(a) · Personnel Requirements. Child care center personnel shall be competent to provide the services necessary to meet the individual needs of children in care and shall at all times be employed in numbers sufficient to meet those needs.
Assistant Director states that since the incident, they have addressed the staff that is in charge of diaper changing and she has been trained accordingly. Communication between teachers and diaper changing staff is timely to ensure that the situation does not recur. Based on interviews, on 11/2/2022, due to lack of communication and/or training, staff did not timely change a child's diaper prior to pick up from the facility, resulting in leakage and wet clothing. This is a potential risk to the health and safety of children in care.

Director will submit a letter to parents informing them that face masks are mandatory. Letter to be distributed to parents on or before 10/4/21. Director will also make disposable masks available to the children upon entering the classroom if needed. A copy of this letter and photo proof of masks for children will be provided to the Department by 10/4/21. Based on LPA's interview with the director and observation of 51 children indoors who were not wearing face coverings and Licensee not enforcing the use of face masks/coverings of children. This is an immediate hazard to the health of children due to the COVID-19 pandemic which is currently on the rise.
