Site director stated that she will walk through all the classrooms and ensure that heavy shelves, aquariums and heavy objects be anchored/braced to the wall. This will be completed no later than 3/24/2025. Director will notify the LPA via email once this is completed. Based on interview and document reviewed, a 3-year old child was taken to emergency and received stitches. A fish tank that was placed on top of play refrigerator fell on top of child. Director will associate staff to preschool program and submit proof by POC date of 10/3/22. Proof will be maintained in staff file. Based on observation and record review.
