Director will have all staff review the "Personal Rights in Child Care Centers" video on ccld.childcarevideos.org and submit a signed attendance sheet of all staff that reviewed the video and send to LPA by email by POC dated 5/15/2025. Based on interviews and record review, the licensee did not comply with the section cited above in when C1 was pulling away from S1 his elbow was dislocated which poses/posed a immediate health, safety or personal rights risk to persons in care. LPA observed water fountain has been permanently removed since test result. Deficiency was cleared during inspection and Letter of Clearance provided. Based on Lead Testing Sample results the facility has one outdoor water fountain with lead exposure. Director stated children used it for drinking water until March 2020 and not been used since then.
